Life With Diabetes

Diabetes Self Management Education

Our Diabetes Self-Management Education Program, accredited by the Association of Diabetes Care and Education Specialists, may be right for you. For those who never received formal diabetes education or who are newly diagnosed, the initial program provides 10 hours of education in a small group setting. The education plan is customized to meet your educational needs. We start with a one on one meeting to determine your needs. This is followed by group sessions on the following Healthy Habits:

  • Healthy Coping
  • Healthy Eating
  • Being Active
  • Taking Medication
  • Monitoring
  • Problem Solving
  • Reducing Risks

Diabetes Self Management Education Follow Ups

For those who have completed the 10 hour education series, the follow up sessions are given quarterly. We will monitor your progress. At each session, we will record your weight, BMI, blood sugar, and blood pressure. You can have a one on one session with our pharmacist or you can choose to repeat one of the classes about Healthy Habits. If you are interested, please contact us.

Diabetes Self Management Education Device Training

These classes are for those patients receiving a continuous glucose monitor such as Dexcom or Freestyle. This program series will educate you on how to use the device and how to use the information that it provides. If you are interested, please contact us.
